Your reglaze job will be carried out in our lab using the information on your actual optician supplied prescription that you send to us with your frame(s).

Prescription Details
Please fill in the form below with the information from the prescription obtained from your opticians.
Right Eye
Right Sphere
Sphere will always be a '+' OR a '-' figure - some opticians write this above the number. It should appear something like this '+2.50'. If the optician has put a symbol that looks like a figure '8' on it's side or has put in plano, it means that the sphere is 0.00.
Right Cylinder
Cylinder will always be a '+' OR a '-' figure - some opticians write this above the number. It should appear something like this '+1.75'. If the optician has written'DS' or has not entered a figure then simply leave this blank.
Right Axis
Axis will be a number between 0-180, there is no '+' or '-' to enter here. Carefully enter the number in the box provided. Note that if there is no Cylinder on the prescription there will also be no Axis - so leave it blank.
Right Near Addition
Your optician may have written 'Add' or 'Near' on your prescription. If your optician has entered a figure for 'Addition' or 'Add' or 'Near' enter it in the box. It is always a '+' figure and relate to an adjustment for near reading use. It is usually the same for both eyes. If nothing has been entered then please leave this blank.
Right Prism
Enter your Prism values. Some lens types may cost extra with a prism.
Right Base
This lets us know where to put the prism in your glasses.
Left Eye

Prescription Date*
Your prescription must be dated within the last 2 years. If your prescription is older than 2 years then we advise you to get an up-to-date one.

So we can find out where to position your new bifocals, we will need you to do one of the following. Please select your choice.
PD Options*
- The frame I am sending to be reglazed is already fitted with bifocal lenses.
- The frame I am sending is not fitted with bifocal lenses but I can also send an old pair of bifocals for you to measure.
- I will send you a passport style photo of me wearing the frame I would like reglazing.

Now you can choose the options you would like for your new lenses. Please note that certain marked options will affect the total order cost.
What's Your Frame Type?*
- Fully Rimmed
- Semi Rimmed
- Rimless
Fully Rimmed Traditional glasses type where the lenses are completely enclosed by the frame.

Semi Rimmed These glasses have a solid frame across the top with a thin vinyl cord wrapped under the bottom of the lenses.

Rimless These are glasses where there is no solid frame and the lenses are mounted directly to the bridge and the temples.

Anti-Scratch Provides a much harder surface thus giving more protection against scratching.

Anti-Reflection Eliminates reflections from the front and back surfaces of the lens. Provides better vision for night driving and are more comfortable for reading and computer use.

Ultra-Clean Not only an anti-reflection coat, it also protects against scratches, smudges, dust, water & UV.

Blue-V Blue light is emmitted from devices that most of us use on a daily basis - mobile phones, tablets, computers and laptops. Blue-V is pioneering technology that cuts out blue light. A unique anti-reflective coating reflects unwanted blue light allowing clearer and sharper vision with improved visual comfort and performance.
